There are several different causes for having a flat foot. Largely it can divided in those that affect us when we are adolescents which rarely needs treatment, or affect us as we get older.
As we get older the ligaments can loosen in combination with weakness of the supporting tendon, causing the foot to collapse further. Most cases can be settled with good arch supportive footwear and physiotherapy to stretch the achilles and strengthen the weakened muscles, but in those that have persistent pain surgery is an option. Most of the time we can correct the foot shape by a combination of minimally invasive bone cuts and screws, titanium wedges in bone cuts to correct the foot alignment, along with moving a tendon to support the foot. Sometimes the arthritis is too advanced and a joint fusion is required to hold the bone in the correct position and assist with pain relief.
